To register, all new patients must attend the Surgery in person between either 9am and 12pm or 2pm to 5pm and produce the following information: Proof of identity (e.g. Passport or new photo ID Driving Licence) Proof of address (e.g. Utility Bill, Bank / Benefit Statement) Should you wish to complete the required New Patient Registration Form prior to registration (which will save you time at the reception desk), the documents can be downloaded from this page. All new patients accepted onto the practice list will be given an appointment for a new patient check with the Practice Nurse. Attending this appointment is a mandatory requirement of registration. We would very much appreciate your help in keeping your details up to date by completing this questionnaire. By providing this information you can help us identify any risk factors to your health. We can then assist you in addressing these and help you stay in good health. Our Lifestyle Questionnaire can be downloaded from this page (see link on the left). The questionnaire may be emailed as an attachment to: Or hand to reception staff when you next visit the surgery.
